This fourth edition of Ellenberg and Rifkin’s DiabetesMellitus: Theory and Practice has largely been rewritten. Many new chapters have been added and some from the previous edition deleted. A survey of the table of contents indicates that several new important areas of information have led us to select additional coverage. In fact, given the enormous pace of progress, we appreciate the publisher’s ability to rapidly translate manuscript into text so that the material is timely.
The book is organized into two major areas. Chapters 1-20 cover basic physiology of metabolism and pathophysiology of diabetes and its complications, and Chapters 21-60 discuss clinical diagnosis, epidemiology, and treatment of diabetes and its complications. All of the chapters provide an up-to-date review of an area by an international group of recognized authorities.
The revolution in molecular and cellular biology has led to great change in our basic understanding of carbohydrate and protein metabolism, and this is reflected in major revisions in the first two chapters of this textbook. The machinery of the endocrine pancreas has been emphasized in new chapters on В-cells and А-cells. Insulin action is now discussed at the molecular level, and much of this new understanding has been put into physiologic and clinical perspective in the chapter on insulin resistance in man. New findings of potentially great significance have led to a chapter on gut and brain peptides. Significant biochemical, pathophysiologic, histologic, and functional studies are the basis for new chapters on renal disease, neuropathy, eye complications, and vascular disease. Because of the importance of animal models to understanding the potential etiology of diabetes, there is a comprehensive and up-to-date new review of diabetes in animals in which potenial relationships to human diabetes are fully discussed. The separation of diabetes mellitus into type I, or IDDM, and type II, or NIDDM, is emphasized by entirely new chapters on the pathophysiology of these forms of diabetes. The clinical aspects of diabetes diagnosis and treatment have been expanded to include chapters on mechanisms and management of brittle diabetes; hyperlipidemia in diabetes; and the mother in pregnancies complicated by diabetes, including gestational diabetes. New chapters also include a discussion of the behavioral, economic, and social aspects of diabetes; the use of computer technologies in diabetes care and education; and the relationship of metabolic control of diabetes to the development and progression of long-term complications. The reality of self-blood glucose monitoring, pumps, and other devices are fully described in separate chapters. New recommendations for nutritional management are given in a chapter which covers both behavioral and traditional nutritional approaches and discusses the most recent recommendations of the American Diabetes Association. An exhaustive, expanded chapter updates the metabolic implications of exercise and physical fitness in the management of the diabetic patient. A separate chapter on hypoglycemia in type I diabetes provides a lucid understanding of the application of human physiology to an increasingly important clinical problem. Two new chapters review the current status of whole and segmental pancreas transplantation and various approaches to islet cell transplantation.
Much new knowledge of both theoretical and practical importance is included. Real progress in the field is being made rapidly. We hope that much of the information from this edition will in its own way contribute to improved understanding and treatment of this protean disease.
As is true in all human endeavors, change has occurred in both the authors and the editors. We are saddened by the loss of one of the founding editors, Max Ellenberg, and would like to acknowledge our great debt to him for his pioneering contributions. We hope that this present edition would please him; we have no doubt that he would be enthusiastic and supportive. We have been pleased by the hard effort expended by the authors in keeping the information brief but complete. It indicates true dedication to education and scholarly pursuits.
